Medical Social Worker (MSW) – Home Health (Part Time)

Health Link Home Health and Hospice is a leading provider of comprehensive home health services across the San Francisco Bay Area and East Bay for over 20 years. We are seeking a compassionate, organized, and team-oriented Part-Time Medical Social Worker (MSW) to serve patients in Alameda County communities.

Responsibilities

  • Conduct psychosocial assessments during patient admission
  • Evaluate social, emotional, and environmental factors impacting care
  • Collaborate on interdisciplinary care plans
  • Provide counseling, resource linkage, and discharge planning support
  • Consult with physicians and care teams regarding patient needs and care adjustments
  • Maintain timely and accurate clinical documentation
  • Supervise Social Work Assistants as needed

Requirements

  • Current California Licensed Clinical Social Worker (LCSW) license
  • Valid California Driver’s License
  • Reliable transportation for home visits
  • CPR and First Aid certification
  • Ability to travel throughout Walnut Creek and surrounding East Bay areas
