Accelerated Home Health Care is hiring a Medical Social Worker (MSW) to provide in-home psychosocial support to patients and families.

Key Responsibilities:

Conduct assessments of psychosocial needs.
Provide counseling and emotional support.
Connect patients and families to community resources.
Document interventions and communicate with the care team.
Follow care plans and regulatory requirements.

Minimum Certification & Licensing Requirements:

MSW degree and state licensure.
Completion of agency onboarding/credentialing.
Up-to-date TB test and reliable transportation.

Preferred Qualifications:

Experience in home health or hospital settings.
Strong interpersonal and problem-solving skills.
Ability to work independently in a home-based setting.

Working Conditions:

In-home visits with travel between patients.
Flexible schedule, including occasional weekends.
Must be able to handle in-home work safely and independently.
