Location : Chilton Medical Center, Pompton Plains, NJ Position : MSW Social Worker Start Date : May 12, 2025 End Date : August 15, 2025 Schedule : Monday – Friday, 8:30 AM – 4:30 PM (Weekends as needed)

In this role, you’ll collaborate with a multidisciplinary team to provide patient-centered care and address social determinants of health (SDOH). Key Responsibilities:
- 60% – Conducting patient assessments, making referrals, and delivering interventions to ensure care needs are met.
- 20% – Documenting interactions and maintaining thorough, accurate records.
- 10% – Participating in meetings, trainings, and contributing to program development.
- 10% – Collaborating with internal and external agencies to improve patient care coordination.
What You’ll Bring:
- Experience : 3–5 years of experience in a health-related field (hospital experience is a plus!)
- Skills : Strong understanding of social services and community resources, with excellent organizational, communication, and interpersonal skills.
- Knowledge : Familiarity with Epic EMR preferred; proficiency in Microsoft Office and EMR documentation standards.
- Licensure : Active LSW license in New Jersey (in good standing).
- Bonus : Bilingual skills and experience in behavioral health are a plus!
