Partners in Home Care is seeking traveler Physical Therapists! Come work with Missoula's premier home health employer. Partners in Home Care is a nationally accredited non-profit organization that has been providing quality home health care services to patients and families throughout Western Montana since 1987. We are passionately committed to assisting patients and families in achieving optimal health, independence, and comfort through high quality, cost-effective home and community health-related services. POSITION: PT Home Health LOCATION/S: Missoula, Missoula County | Hamilton and Stevensville, Ravalli County | Ronan and Polson, Lake County STATUS: Full-Time | 40 hours/week possible call SCHEDULE: Typically Monday through Friday, 8am-5pm. This position participates in rotating on-call, weekend, and holiday coverage. Job Summary: The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education. Essential Duties and Responsibilities: 1. Evaluates and establishes plan of care to meet patients needs in the home environment. Coordinates care with other disciplines and seeks orders for appropriate additional services. 2. Provides direct patient care based on the established care plan and physician orders. 3. Re-evaluates patient status and progress. Modifies treatment as appropriate, including frequency and duration of care. 4. Communicates appropriately with physicians and other staff members. 5. Documents all skilled services provided to patients. 6. Communicates with scheduler on each day worked. 7. Maximizes consistency of care when preparing schedules. 8. Oversee services provided by Physical Therapist Assistant (if any). 9. Performs other duties as assigned by supervisor. Minimum Qualifications: Qualifications include BS degree or better from an accredited Physical Therapy program with current license to practice in the State ofMontana, valid driver s license, and appropriate auto insurance. Two years of previous PT experience preferred.
