RN Care Manager

About the RoleAre you a registered nurse who thrives on building relationships with patients and helping them manage complex chronic conditions? Thundermist Health Center is looking for an RN Care Manager to carry a caseload of moderate to high-risk adults - providing comprehensive screening, assessment, disease education, self-management support, and care coordination in a community primary care setting. You'll work alongside primary care providers to close care gaps, connect patients to community resources, and empower individuals to take an active role in their own health. Manage a caseload of moderate to high-risk adults with complex chronic health conditionsEstablish individualized care plans with goals, interventions, and contact schedules based on risk category and each patient's medical and social needsPromote compliance with disease-specific clinical outcomes by providing self-management support including disease education, medication charts and side effects, signs and symptoms to watch for, nutritional recommendations, and exercise guidanceConnect patients with community supports, services, and resources including pharmacy, DME, and home care providersCoordinate sick visits and additional services as needed

Collaboration & Communication

Work collaboratively with primary care providers, the health care team, and community agencies to coordinate care and disease managementUse all available methods of communication with patients, including interpreting services, counseling, group discussion, written materials, and visual aidsIdentify and communicate cultural, social, and economic characteristics of the patient population servedMaintain clear, consistent communication with your supervisor, providers, and other staff members

Documentation & Quality

Document clearly and concisely in the medical recordActively participate in performance improvement activities to improve quality of care and safetyAssist with clinical audits and database management as assignedMaintain OSHA and HIPAA guidelines and adhere to organizational policies and procedures

What We're Looking ForYou're a nurse who sees the whole patient - not just the diagnosis. You're organized enough to manage a complex caseload, clinical enough to interpret labs and educate on medication regimens, and compassionate enough to sit with a patient who's struggling and help them find a path forward. You bring professionalism, leadership, triage skills, and genuine warmth.

Current State of Rhode Island RN licenseCertified Diabetes Outreach Educator (CDOE) within one year of hireCPR/BLS certificationExperience working with primary care providers to coordinate care and disease managementKnowledge of disease management, case management, and chronic care principlesExceptional oral and written communication skills with proficiency in medical terminologyA genuine commitment to patient-centered, high-quality care for diverse and underserved populations, with an authentic passion for health equity and community health

Preferred:

BSN preferredExperience in community health, public health, chronic disease management, community nursing, or case managementCertification in Case Management, Diabetes Education, Asthma/COPD, or Cardiovascular care

Schedule & LocationsThis is a full-time (40 hours/week) salaried position. Schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., based at a Thundermist location.
