Post Acute Primary Care NP/PA Needed! A growing post-acute care practice in the Syracuse, New York area is seeking a Nurse Practitioner or Physician Assistant to join a collaborative, well-supported team. This is a full-time opportunity (5 days/week) providing primary care services in skilled nursing facilities, offering strong clinical autonomy, structured onboarding, and uncapped earning potential in a mission-driven environment. You’ll care for a diverse patient population, work closely with physicians and interdisciplinary teams, and play a meaningful role in improving outcomes during critical transitions of care.
