About the Role

Our NP will perform comprehensive health assessment which include but not limited to the following:

  • Obtain history and perform physical examination

  • Complete review of medicines and related instructions

  • Review home safety and psychosocial needs

  • Fulfill documentation needs for STAR and HCC measures

  • Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions

  • Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies

  • Maintain patient safety and confidentiality

  • Universal precautions utilized in all patient care

  • Prescribe/order procedures, treatments and medications within scope of practice

  • Develop high level of trust and intimacy with patients and families

The Skills, Experience & Education You Bring

  • CA Licensed Nurse Practitioner

  • 2+ years of Palliative Care experience

The Base Salary for this position is$128,700 - $154,440/year

Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
