Job Overview
We are seeking an experienced Nurse Practitioner for a part-time opportunity serving patients in Emporia and surrounding Southside Virginia communities. This position provides primary care and post-hospital follow-up services in patients' homes and senior living settings. The schedule is 3 days per week, offering flexibility while supporting continuity of care for patients transitioning from the hospital or managing ongoing health needs.
Role Overview
The Nurse Practitioner will independently assess and manage patients requiring primary, transitional, and post-discharge care. Responsibilities include evaluating acute and chronic conditions, monitoring recovery following hospitalization, developing individualized treatment plans, and coordinating with caregivers and healthcare professionals to promote positive outcomes and reduce preventable hospital readmissions.
Key Responsibilities
- Deliver primary care and post-hospital follow-up services in patient homes and senior living communities
- Perform comprehensive health assessments and follow-up evaluations
- Conduct post-discharge visits and support safe transitions from hospital to home
- Evaluate, diagnose, and manage acute and chronic medical conditions
- Develop and update individualized treatment and care plans
- Prescribe medications and order clinically appropriate diagnostic testing
- Coordinate specialty referrals and collaborate with interdisciplinary healthcare teams
- Educate patients and caregivers on medications, disease management, preventive care, and treatment plans
- Monitor patient progress and modify treatment plans as clinically appropriate
- Complete EMR documentation within 24 hours of each patient encounter
- Consult with the Medical Director or collaborating providers when additional clinical guidance is needed
Schedule
- Part-time: 3 days per week
- Flexible scheduling
- Typical hours: 8:00 AM–5:00 PM
- No on-call responsibilities
- Orientation and onboarding provided
- Credentialing generally takes approximately 30 days

Required Qualifications
- Active Virginia Nurse Practitioner license
- Current DEA registration
- Active CAQH profile and CAQH number
- Minimum of one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care
- Experience utilizing electronic medical record (EMR) systems
- Strong clinical assessment abilities with the capacity to make independent clinical decisions
Preferred Qualifications
- Experience in post-acute or transitional care
- Clinical background in urgent care, emergency medicine, hospital medicine, or acute care
- Experience caring for geriatric or medically complex patient populations
- Previous experience delivering care in patient homes or senior living environments
