Position Summary
A well-established outpatient clinic in Chino Valley, AZ is seeking a full-time Nurse Practitioner or Physician Assistant to join its dedicated primary care team. This is an excellent opportunity for a provider who values continuity, collaboration, and compassionate care. The role offers a consistent weekday schedule, manageable patient volume, and strong administrative support—including a dedicated scribe to streamline documentation.

Compensation & Benefits
- Base Salary: $120,000 – $150,000 annually (dependent on experience)
- Incentive Bonus: Performance-based structure

Qualifications
- Current and unrestricted license in Arizona as a Nurse Practitioner or Physician Assistant
- DEA registration or eligibility to obtain in Arizona
- Clean professional background (no disciplinary actions or malpractice claims)
- Credentialing eligibility with Medicare, Medicaid, and commercial insurers
- Experience in outpatient primary care or family medicine preferred
Key Responsibilities
- Perform wellness exams, physicals, and preventive screenings
- Diagnose and treat acute and chronic medical conditions
- Create and manage personalized care plans
- Order, interpret, and follow up on labs and imaging
- Educate patients on lifestyle changes and disease prevention
- Collaborate with a medical scribe for accurate clinical documentation
- Coordinate referrals and manage care transitions
- Uphold clinical quality standards and best practices
