Job Description
Advanced Practice Clinician (Nurse Practitioner / Physician Assistant)
Hours: 40 per week Duration: Contract
About the Role
The Advanced Practice Clinician (APC) will support the Senior Community Care (SCC) program, a longitudinal and integrated care model focused on delivering high‑quality clinical services to members in facility-based settings. This role provides care to high‑risk health plan members as part of an interdisciplinary team.
This position offers strong work–life balance with flexible scheduling, no on‑call responsibilities, and no weekend or holiday requirements. Optum, one of the largest employers of APCs, provides extensive opportunities for professional growth and career development.
Primary Responsibilities
Conduct comprehensive clinical assessments
Manage acute and chronic medical and behavioral health conditions
Ensure accurate and complete documentation, including ICD‑10 coding
Collaborate with the interdisciplinary care team
Lead advanced illness and care‑planning discussions
Provide education and counseling to patients and caregivers
Required Qualifications
Active, unrestricted state license (or ability to obtain by start date)
NP: Graduate of an accredited MSN or DNP program
PA: Graduate of an accredited Physician Assistant program
Active national certification:
NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
PA: NCCPA certification
Active DEA licensure/prescriptive authority
Ability to meet physical job requirements (lifting 30‑lb bag, navigating stairs, varied environments, ability to sit/stand/kneel for assessments)
Reliable transportation
Ability to obtain a collaborative practice agreement (if required by state)
Full COVID‑19 vaccination (per federal, state, local, and client requirements)
Must pass pre‑employment drug screening (UnitedHealth Group is a drug‑free workplace)
Preferred Qualifications
1+ year of clinical experience (long‑term care preferred)
Experience supporting patients with complex behavioral, social, or functional needs
Knowledge of geriatrics and chronic illness management
Experience with advanced illness and end‑of‑life discussions
Proficiency with electronic and written medical documentation
Schedule & Work Details
Shift: 8:00 AM – 5:00 PM
Days: Monday–Friday
Dress Code: Medical attire

Regions: Hillsborough and Cheshire Counties, New Hampshire
