Your responsibilities include:

Providing comprehensive primary care services to a manageable patient panel of 12-20 patients per day
Collaborating with interdisciplinary teams to optimize patient outcomes
Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications

Graduate degree as a nurse practitioner or physician assistant
Active and unrestricted NP or PA license in North Carolina
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Experience in post-acute care preferred, but not required (training available)

Compensation is fee for service (FFS). Expected compensation is estimated range of $103,000 to $122,000 annually with no cap on productivity income potential.
