At Impact Life, we are seeking a compassionate and innovative Nurse Practitioner or Physician Assistant (NP/PA) to join our Social Prescribing Whole Person Healthcare Program. This role provides an opportunity to deliver integrated primary care while helping patients connect with community resources and social supports that improve overall health outcomes.
This part-time position (20 hours per week) is on-site at our Newark, Delaware location. The Social Prescribing site operates Monday through Friday from 8:00 AM to 4:00 PM, and the provider’s 20 hours per week will be scheduled within those operating hours.
What You Will Do
- Conduct comprehensive assessments of patients’ physical, mental, and social health needs
- Provide primary care services including diagnosis, treatment, and preventative care
- Develop individualized care plans that integrate medical care with social prescribing interventions
- Prescribe medications and manage treatment plans in accordance with Delaware licensure and regulatory requirements
- Promote health literacy and patient self-management strategies by actively engaging patients and their families
- Collaborate with a multidisciplinary team of healthcare professionals to coordinate and improve care delivery
- Address social determinants of health by connecting patients with community resources and supportive services
- Provide education and advocacy about the benefits of social prescribing and whole-person care
Requirements
Qualifications
Required
- Master’s Degree in Nursing from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
- Current and valid Nurse Practitioner or Physician Assistant license in the State of Delaware
- Certification and credentials that allow for independent practice as a Primary Care Provider in Delaware
- Minimum of 2 years of clinical experience, preferably in primary care, behavioral health, or integrated care settings
- Prescriptive authority with DEA and Delaware CSR registration
Preferred
- Experience working in integrated healthcare or collaborative care models
- Experience with social prescribing models or community resource navigation
- Strong background in behavioral health integration
- Experience working with underserved or vulnerable populations
