Ideal Candidate

A Nurse Practitioner/Physician Assistant seeking to join the Chestnut team on a part-time or 1099 contract basis
8-10 hours available per week, flexible day/time
Interested in physical medicine and rehabilitation (PM&R)

What You’ll Do

Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Collaborate with therapists and the facility’s medical team on treatment plans.

Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.

Requirements

Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice
If an NP: ANCC or AANP certification
Current DEA and state Controlled Substance License (required upon start date)
SNF, pain management, or physiatry experience a plus
