The Duke Department of Obstetrics and Gynecology is seeking an Advanced Practice Provider (APP) to join its team of experienced Complex Menopause providers. The APP will deliver evidence-based care, patient education, and counseling that helps patients navigate midlife health concerns. The team includes generalist OB/GYN physicians with Menopause Society certification who will provide mentorship and clinical oversight.

The schedule is variable and may require flexibility, including early-evening and weekend hours to support patient and clinician scheduling needs.

Primary responsibilities

Provide evaluation, management, education, counseling, and treatment for patients with common and complex menopause-related concerns, including:

• Medical management of menopausal health concerns, including menopausal hormone therapy
• Symptomatic perimenopause, including hormonal contraception, IUD use, and cyclic progestogen therapy
• Menopausal and perimenopausal symptom management for patients with hereditary breast and ovarian cancer risk, high-risk cancer patients, and cancer survivors, using hormonal and non-hormonal approaches as appropriate. Patient population may include natural, premature, early, surgical, and radiation-induced menopause
• Menopausal and perimenopausal symptom management in patients with higher-risk conditions, including tobacco use, clotting disorders, cardiovascular disease, valvular disease, liver disorders, and transplant history
• Assessment of abnormal and postmenopausal bleeding
• Menopause-related cognitive and mood changes
• Genitourinary syndrome of menopause
• Midlife vulvar dermatoses and vulvovaginitis
• Midlife weight management
• Sexual health and sexual dysfunction, including desire, arousal, orgasm, genitourinary, and pelvic floor concerns, and related treatment options

Gynecological Care

Based on menopause patient volume, the APP may provide in-person routine gynecologic care and treatment for common concerns including:

• Family planning and reproductive life-planning counseling
• Routine screening examinations and cervical cancer screening
• Referrals for age- and risk-appropriate screening, including mammography, bone density testing, and colorectal cancer screening
• Screening and treatment for sexually transmitted infections
• Evaluation of breast changes or masses
• Evaluation of pelvic or abdominal pain
• Routine postoperative follow-up and problem visits
• Treatment of vulvar cysts and abscesses

Responsibilities may also include in-person assessment and treatment opportunities such as:

• IUD placement and removal
• Endometrial biopsy
• Vulvar biopsy
• Other in-person gynecologic assessments and treatments within the APP's scope, credentials, and privileges

Job Eligibility Requirements

Education

• Nurse Practitioners: Requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
• Physician Assistants: Requires completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.
• Certified Nurse Midwife: Requires completion of an accredited graduate-level nurse‑midwifery program at the MSN or DNP level.

Certification

• BLS required

License

• Nurse Practitioners: Must have current or compact RN licensure in the state of North Carolina. National certification as a Nurse Practitioner required.
• Physician Assistants: Must be licensed in the state of NC and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.
• Certified Nurse Midwife: Must have current or compact RN licensure in the state of North Carolina. National certification as a Nurse Midwife required.
• DEA license for applicable practice state(s)

The remote care model may support patients across multiple states as program licensure expands. Applicants should identify every state in which they currently hold an active professional license and any compact licensure status, as applicable.

Experience

• Two or more years of experience as a Nurse Practitioner, Physician Assistant or Certified Nurse Midwife in Women's Health
• Experience managing midlife Women's Health concerns
• Experience or training in sexual health management and/or prescribing anti-obesity pharmacotherapy
• Certification by The Menopause Society, or comparable menopause-focused training is preferred or must be licensed within six months of employment
