Overview
Job Description
- Conduct admission, recertification, and post-hospitalization assessments
- Develop, implement, and evaluate individualized patient care plans
- Administer medications and monitor patients for side effects and reactions
- Monitor, document, and report changes in patient condition
- Obtain and coordinate medical orders from physicians, nurse practitioners, and physician assistants
- Provide clinical guidance and troubleshooting support to field nurses and caregivers
- Respond to urgent clinical situations and emergencies in a timely manner
- Complete annual supervisory visits for LPNs, RNs, and CHHAs
- Ensure compliance with DOH regulations, policies, and safety standards
- Address and resolve patient and family concerns or complaints professionally
- Maintain accurate and timely electronic documentation
- Ensure all infection control and safety precautions are properly implemented
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Bachelor’s or Master’s degree in Nursing (BSN preferred)
- Active Registered Nurse (RN) license in the state
- BLS/CPR certification (American Heart Association required)
- ACLS certification required
- Minimum 2 years of clinical nursing experience
- Home care experience strongly preferred
- Strong knowledge of home care regulations and compliance standards
- Excellent communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Proficiency in electronic medical records (EMR) and basic computer skills
- A compassionate leader who prioritizes patient-centered care
- Strong clinical decision-making and problem-solving skills
- Ability to support and mentor field staff
- Professional, reliable, and detail-oriented
