Job Description

Oasis Therapy Services is an agency committed to providing exceptional therapy services to seniors. Our team includes experienced and compassionate physical, occupational, and speech therapists specializing in senior care. We offer rehabilitation, pain management, and cognitive and speech therapy. Our priority is the comfort and well-being of our clients, and we strive to create a welcoming and supportive environment.

Oasis Therapy Services is seeking an Occupational Therapist (OT) to provide one-on-one rehabilitation services at an assisted living facility in Cannon Falls, Minnesota.

Responsibilities

Direct the resident evaluation and reevaluation process.
Develop and document individualized occupational therapy intervention plans.
Provide therapy focused on ADLs, mobility, safety, and independence.
Monitor residents’ progress and modify intervention plans as needed.
Direct and document discharge planning and recommendations.
Educate residents and caregivers on exercises, adaptive techniques, and equipment.
Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

Active and current Minnesota Occupational Therapy license as an Occupational Therapist.
Graduate of an accredited Occupational Therapy program.
Current CPR certification.
Strong communication, organization, and documentation skills.
Ability to work independently and collaborate effectively with residents, staff, and therapy professionals.
Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
