Full-Time, Part-Time, or Per Diem

Health Link Home Health & Hospice is seeking an Occupational Therapist (OT) to support our growing South Bay team.

We provide the structure and operational support therapists need to focus on patient care — organized scheduling, strong clinical coordination, and modern documentation tools.

Your Role

• Perform occupational therapy evaluations and reassessments• Develop and manage individualized plans of care• Deliver skilled therapy focused on ADLs, safety, fall prevention, and independence• Address home safety and adaptive equipment needs• Educate patients and caregivers• Collaborate with physicians and interdisciplinary team members• Complete documentation in Kinnser

Requirements

• Active California Occupational Therapist license• Valid California driver’s license• Strong clinical judgment and time-management skills• Home health experience preferred but not required
