Occupational Therapist (OT) – $42–$45/hr | Full-Time | Skilled Nursing Facility | NY

Blossom Healthcare Center is seeking a dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. This is a great opportunity to work in a collaborative, patient-focused environment where you can make a meaningful impact on residents’ independence and quality of life.

Your Role:

Evaluate residents and develop individualized treatment plans based on medical diagnoses
Implement therapy programs to improve functional independence and daily living skills
Monitor and document resident progress through daily notes, progress reports, and re-certifications
Maintain accurate and compliant documentation of care services
Supervise and provide clinical guidance to Occupational Therapy Assistants (OTAs)
Participate in care plan meetings and interdisciplinary collaboration
Assist with departmental projects, reporting, and data tracking as directed
Ensure all care is delivered in compliance with state, federal, and facility guidelines

Qualifications:

Graduate of an accredited Occupational Therapy program
Active New York State OT License required
Strong clinical, documentation, and communication skills
Experience in skilled nursing or rehab setting preferred
Team-oriented with a commitment to high-quality patient care
