Job Description
Post Rehabilitation Services Civita Care Center at Milford is seeking a licensed Occupational Therapist to join our therapy team in delivering patient-centered rehabilitation services at our skilled nursing facility.
In this role, you will evaluate residents' needs and develop evidence-based treatment plans to support optimal function, independence, and participation in meaningful daily activities.
Primary Responsibilities:
- Perform comprehensive evaluations of residents' physical, cognitive, perceptual, and psychosocial abilities, then design and implement individualized treatment plans focused on activities of daily living (ADLs such as bathing, dressing, grooming, toileting, and feeding) and instrumental ADLs (IADLs such as medication management, home safety, and community participation).
- Guide residents through occupation-based interventions, including adaptive equipment training, energy conservation techniques, cognitive strategies, environmental modifications, and fall prevention education to enhance safety, confidence, and quality of life.
- Monitor resident responses to treatment, document progress accurately in compliance with Medicare and regulatory guidelines, communicate observations and adjustments to the interdisciplinary team, and collaborate with PTs, SLPs, nursing staff, physicians, and families to ensure comprehensive, coordinated care.
