Job Title: Oncology Nurse Navigator

Position Overview
The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.
Key Responsibilities
- Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs
- Advocate for timely, high-quality care by guiding patients through the healthcare system
- Coordinate appointments, referrals, and services to ensure continuous care
- Provide education on treatment plans, symptom management, and health maintenance
- Maintain accurate documentation and tracking of patient progress and clinical outcomes
- Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation
- Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes
- Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life
- Serve as a trusted resource for patients and families throughout the cancer journey
- Perform RN scope duties, including telephone triage and clinical assessments
- Participate in clinic activities and assist in care coordination and planning
- Perform other duties as assigned
Qualifications
Education & Experience
- Graduate of an accredited nursing program (required)
- At least 1 year of nursing experience (required)
- Bachelor’s degree in nursing (preferred)
- Outpatient nursing and scheduling experience (preferred)
Credential/Licensure
- Current RN license in the state of practice or compact license (required)
- BLS certification (required)
- OCN certification required within two (2) years of hire
Skills & Abilities
- Strong knowledge of oncology practices and patient navigation
- Excellent verbal and written communication skills
- Critical thinking and conflict resolution skills
- Ability to prioritize, manage time effectively, and work both independently and collaboratively
- Proficiency with electronic documentation and EMR systems
- Compassionate, patient-centered approach with strong attention to detail
