Occupational Therapist (OT) – PRNAutumn Lake at MadisonMadison, CT$50–$55/hourStern Therapy Consultants is seeking a PRN Occupational Therapist (OT) to join our rehabilitation team. This flexible opportunity allows you to provide high-quality therapy services while maintaining a schedule that fits your availability.Position Responsibilities:Perform occupational therapy evaluations and develop individualized treatment plans

Provide treatment focused on ADLs, upper extremity function, cognition, coordination, balance, and safety awareness

Recommend adaptive equipment and therapeutic strategies to maximize resident independence

Collaborate with nursing, therapy staff, physicians, and families to support resident goals

Monitor progress and adjust treatment plans as needed

Complete timely and accurate documentation
