Occupational Therapist (OT) – PRNThe Elms Center | Milford, NH$50–$55/hour | PRNStern Therapy Consultants is looking for a PRN Occupational Therapist at The Elms Center in Milford, NH. Use your clinical skills to help skilled nursing residents rebuild daily routines, adjust to changing abilities, and work toward meaningful goals during rehabilitation.What You'll DoAssess how strength, coordination, cognition, and endurance affect residents' ability to manage everyday tasks.Establish individualized plans of care with measurable goals for self-care and functional participation.Deliver treatment through practical activities such as dressing, grooming, bathing, and toileting.Identify adaptive equipment and environmental modifications that support safer task performance.Educate residents and caregivers on strategies they can use between sessions and after discharge.Review progress, oversee COTA services within the treatment plan, and communicate recommendations through documentation and care team discussions.What We Offer$50–$55 per hour.PRN coverage opportunities aligned with your availability and facility needs.Unlimited referral bonus, with up to $1,000 per successful referral.Collaboration with fellow clinicians and access to therapy leadership for support.QualificationsActive New Hampshire Occupational Therapist license.Sound clinical judgment and strong treatment planning skills.Clear communication and accurate, timely documentation.Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.Apply today to provide occupational therapy care at The Elms Center with Stern Therapy Consultants.
