Job Description
- Education: High School Diploma or equivalent required; Degree in social work, public health, human services, or related field preferred, educational equivalent may be considered with 2+ years of related experience.
- Licenses: Valid state-issued driver’s license required; Montana Certified Application Counselor license or ability to obtain within six months; BLS CPR certification or ability to obtain within 30 day.
- Experience: Prior experience in a healthcare setting or care management role preferred; experience with health insurance administration and enrollment highly preferred; experience serving a diverse population including those who are vulnerable and/or may have high needs preferred; previous EHR experience preferred.
- Operates a computer system and related software to complete work assignments.
- Possess and apply the customer service skills necessary to effectively address the needs of the patients, regardless of their special needs.
- Perform duties in a manner that is both professional and compassionate.
- Apply knowledge in a manner that allows the needs each patient to be met.
- Follow Blacktail Health’s established policies and procedures.
- Serve as a central resource for community questions and referrals for both consumers and providers, building relationships with community resources, public health, schools, churches, and other social service organizations to identify, refer, and develop resources that remove consumer barriers to accessing needed health and social services.
- Assist eligible clients to apply for health insurance plans, providing necessary follow-up, tracking and data collection to ensure continuous enrollment.
- Develop and maintain a tracking and follow-up system for applications to ensure that all aspects of the process, from application through post-enrollment, have been completed and that the client is able to receive appropriate services.
- Maintain statistics/data on contacts; provide reporting, identify needs, problems, and service gaps; track and tabulate contacts, application submission and post-enrollment follow-up.
- Initiate post-enrollment support activities and record information required for monthly and quarterly reports.
- Attend appropriate community or networking meetings to facilitate outreach and gathering of information to increase consumer access to a permanent medical home.
- Assist in developing educational materials to distribute through local community groups.
- Assist in organizing and holding external education and enrollment events to increase access to our services in the community.
- Travel as needed to assist with outreach and enrollment activities.
- Coordinate with other departments for marketing campaigns to increase awareness in the community of Open Enrollment, deadlines, and opportunities for health insurance enrollment.
- Support and contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities.
- General duties as needed.
- Other related duties as assigned that support organizational goals and objectives and patient-centered care.
- Work in conjunction with Blacktail Health providers and staff to provide efficient and high-quality direct services related to insurance enrollment.
- Follows established policies and procedures regarding electronic health record (EHR) documentation of communication, visits, services provided, and plans for future care.
- Daily internal referral reviews using the EHR and patient contact for appointment scheduling.
- Monitor and respond to pager (device) and overhead paging for requests for enrollment services.
- Participate in population health management through collaborative meetings, departmental support, and specific “clinic” care
- Participate in department meetings to facilitate communication, skill and knowledge advancement, workflow development, and other important aspects of job duties.
- Attend community-based meetings (as assigned) with representatives from external resources and services for information sharing, partnering, and targeting population health needs.
- Take responsibility to learn new position responsibilities as needed, including workflow changes and developments in technology.
- Professionally take direction or re-direction on work performance and strive to improve performance as needed.
- Attend training and integrate training materials into work performance as approved by supervisor.
- Assist Outreach and Enrollment Supervisor with duties as required and as assigned.
- Ask for help from others as needed.
- Maintain a calm, respectful, and professional demeanor in all team and patient contact.
- Keep other team members informed of whereabouts when out of the clinic during work time.
- Other duties as assigned.
- Competency in dealing with diverse populations; particularly with high need and/or vulnerable populations.
- Excellent knowledge and understanding of Medicare and Medicaid, health insurance options, health insurance exchanges, and subsidy programs and ability to communicate this information effectively to varied audiences.
- Ability to protect the dignity, privacy, and confidentiality of patients and families and patient/client’s right to self-determination.
- Ability to work independently and as a member of a team.
- Excellent oral and written communication skills.
