P/T Patient Care Coordinator

As a Patient Care Coordinator, you will:

• Follow up on Face-to-face requirements, educating physicians on CMS guidelines for Home Health in person, or virtually by email or phone.
• Visit different VA offices in NKY/Cincinnati area to obtain orders/signatures and follow up on VA patients.
• Attend Tuesday business development meetings, bringing relevant data, reports, and information about changes within accounts and referral sources.
• Report to a Market Executive of Sales
• Work an 8 hour shift every Tuesday/Thursday or Tuesday/Friday

Required Experience/Skills:

• RN/LPN/LVN license.
• Three years of nursing experience is preferred
• Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
• We require travel within an assigned territory.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$59,300 - $80,900 per year
