The PACE Registered Nurse (RN) Supervisor provides clinical leadership and operational oversight for all PACE-related nursing services, including home-based skilled nursing care, case management and ambulatory care. This role ensures the delivery of high-quality, participant-centered care across the continuum, while supporting, supervising, and developing nursing staff in alignment with PACE regulations, organizational policies, and interdisciplinary care model standards. The RN Supervisor balances direct clinical care, clinical supervision, and operational coordination, serving as a clinical resource for the interdisciplinary team (IDT). The role emphasizes care coordination, staff competency, quality improvement, and participant satisfaction.

Essential Responsibilities:

  • Provide direct or indirect nursing care to participants in their homes or care settings. Including home-based and ambulatory case management and assessments, particularly during periods of program growth, cross coverage, and scaling operations.

  • Serve as a clinical resource for complex participant care situations, including high-risk transitions, acute changes in condition, and escalation of care needs.

  • Attend IDT meetings serving as a member, facilitator, or leader as indicated to provide nursing leadership expertise, guide decision making, and support the team in participant-centered care.

  • Provide day-to-day clinical supervision and support to nurses, personal care attendants, and other assigned nursing staff in the PACE Center and in community settings.

  • Accountability for medication and immunization administration and/or oversight in the PACE Center and community/home-based settings.

  • Implementing controls and monitoring to ensure adherence to the PACE Infection Control Plan

  • Develop, implement, and coordinate individualized care plans for participants

  • Assess participants' conditions, perform diagnostic testing under scope of licensure, start intravenous lines, and perform nursing-based assessments.

  • Communicate and educate participants and their caregivers emphasizing Age Friendly care.

  • Accurately document in the Electronic Health Record (EHR) in a timely manner.

  • Serve on various internal and external committees as required or designated.

  • Participate in team meetings, caregiver groups, and special events as needed.

  • Provide on-call coverage for direct patient care with other staff as needed.

  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.

  • Conduct hiring, onboarding, coaching, performance evaluations, and corrective action as needed, consistent with HR policies to direct reports.

  • Foster a culture of accountability, service excellence, and continuous improvement.

  • Promote adherence to evidence-based nursing practice, PACE regulatory requirements, and organizational policies and procedures.

  • Support onboarding, mentoring, coaching, and competency validation for nursing staff, particularly during periods of program expansion.

Supporting Responsibilities:

  • Meet department and company performance and attendance expectations.

  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.

  • Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: Minimum of 5 years of progressively responsible nursing experience in PACE, acute care, ambulatory, long-term care, home health, or comparable clinical setting is required. Additionally, at least 1 year of direct clinical experience working with frail, elderly, or medically complex older adults is required. Prior nursing leadership or supervisory experience (e.g., charge nurse, nurse supervisor, nurse manager, or similar role) is strongly preferred. Experience working in a PACE program, geriatric-focused model of care, managed care environment, or interdisciplinary team-based care setting is preferred.

Education, Certificates, Licenses: A Registered Nurse with a current, unrestricted Oregon state license. Valid CPR certification (BLS or ACLS acceptable) required at time of hire.

Knowledge: Possesses thorough knowledge of community services, providers, vendors, and facilities available to assist members. Demonstrates the ability to use computerized systems for data and document recording and retrieval. Maintains a current clinical knowledge base. Exhibits excellent interpersonal and communication skills. Capable of working independently and as part of a team. Proficient in electronic medical records systems.

Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:

  • Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.

  • Only act within the scope of his or her authority to practice.

  • Have 1 year of experience working with a frail or elderly population

  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.

  • Be medically cleared for communicable diseases before engaging in direct participant contact.

Environment: Work inside in general office and clinical settings with ergonomically configured equipment. Travel is required approximately 10% of the time.

Physical Requirements: Work is performed in a clinic setting with typical healthcare exposures, including contact with bodily fluids, infectious diseases, and cleaning chemicals; PPE is provided and required. The role involves stooping, bending, sitting, standing, and moving between clinical areas for extended periods. May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics. Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment. Some positions require occasional travel to PACE sites, participant homes, or external appointments. Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.
