Our wonderful nursing staff meets our patients and families where they are, private homes, nursing facilities, assisted living facilities, and our freestanding 24-bed inpatient center. Do you have the Hospice Heart?Your life as a Palliative Care Social WorkerYour role is very important to our patients and organization. We meet our patients where they are and this position will serve our patients in Wichita, Archer, Clay, Jack Montague, and Young counties. We will rely on you to ensure that we support our staff in making sure that the physical, psychosocial, spiritual concerns/problems of patients and families are understood. We work as a team so there will be a need to interact with other members of our team to provide support to the physician and other IDT members in recognizing and understanding the psychosocial factors that may exacerbate the symptoms related to terminal illness and collaborate on the planning of such care. This schedule is from 8:00 am to 5:00 pm Monday thru Friday.Job QualificationsHas a Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education; orBachelor's degree in social work required. Degree from a school of social work accredited by the Council on Social Work Education required and is supervised by an MSW (as above #1).Current certification as a licensed social worker from the Texas Department of Human Services.Hospice experience preferredLBSW must have 1 year of Clinical experience Ability to understand the physical, psychosocial, and spiritual concerns/problems of chronically and terminally ill patients and families.Ability to conceptualize, interpret and integrate social work practice into the comprehensive palliative care approach to patient/family care.Ability to work effectively within an interdisciplinary team.Demonstrate problem solving skills.Demonstrate competency as defined.Ability to advance in computer knowledge.Ability to be at work as scheduled and alert/awake.Ability to relate well with patients, families, community at large and health care professionals.Ability to communicate effectively both orally and in writing.Ability to initiate your own workload.Demonstrate flexibility and ability to handle multiple demands.Possess clear, empathic communication skills, both oral and written.
