*This position is Part-Time, 16-24 hours per week

At Cura of Melrose, we are looking for a compassionate, dependable, and dedicated Pasrt Time Social Worker or Social Services Designee to join our interdisciplinary care team. As a Social Worker, you will play an important role in supporting the psychosocial well-being, dignity, and quality of life of our residents. You will work closely with residents, families, and the interdisciplinary team to identify individual needs, provide support and resources, assist with care planning and discharge planning, and advocate for resident preferences and rights.

The ideal candidate is compassionate, organized, and committed to providing resident-centered support throughout each resident’s stay. This position requires strong communication and problem-solving skills, the ability to navigate sensitive situations professionally, and a passion for supporting residents and their families.

Responsibilities

  • Complete required psychosocial assessments and documentation for residents in accordance with facility, state, and federal requirements.
  • Identify residents’ psychosocial, emotional, behavioral, and social needs and develop appropriate interventions and approaches.
  • Participate in interdisciplinary care planning and advocate for resident preferences, goals, dignity, and quality of life.
  • Provide support, education, and resources to residents and families throughout admission, stay, discharge, and transitions of care.
  • Assist with discharge planning and assist residents and families in identifying appropriate services, equipment, resources, and placement options.
  • Assist with admissions and transitions into the facility to promote a positive resident and family experience.
  • Serve as a resource for residents and families regarding resident rights, advance directives, healthcare decision-making, and available services.
  • Assist residents and families with adjustment to long-term care, changes in health status, grief, loss, and other psychosocial concerns.
  • Participate in resident and family meetings and help address concerns, grievances, and care-related questions.
  • Monitor residents for changes in psychosocial well-being and communicate concerns to the appropriate members of the interdisciplinary team.
  • Support behavioral health needs and collaborate with nursing, providers, and other disciplines regarding appropriate interventions and services.
  • Maintain accurate and timely documentation in the electronic health record.
  • Assist with required assessments, care plan documentation, and other Social Services documentation.
  • Promote and protect resident rights, privacy, dignity, choice, and self-determination.
  • Maintain confidentiality and comply with HIPAA and all applicable federal, state, local, and facility requirements.
  • Participate in quality improvement initiatives, survey readiness, resident-centered initiatives, and interdisciplinary meetings.
  • Maintain professional communication with residents, families, staff, providers, and outside agencies.
  • Promote a culture of compassion, professionalism, teamwork, and resident-centered care.
  • Perform other duties as assigned to support the Social Services department and the overall needs of residents.
Requirements
    • Bachelors degree in related field preferred
    • Previous experience in long-term care, skilled nursing, healthcare, or a related setting preferred.
    • Knowledge of resident rights, psychosocial needs, discharge planning, and community resources.
    • Strong communication, interpersonal, organizational, and problem-solving skills.
    • Ability to work effectively as part of an interdisciplinary team.
    • Ability to communicate professionally and compassionately with residents, families, staff, and outside providers.
    • Knowledge of HIPAA and applicable state and federal long-term care regulations preferred.
