Social Worker Responsibility Highlights include:

- Meeting with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, residents rights and discharge planning. Implement documents as appropriate with residents and/or their responsible parties.

-Conduct and document social service evaluations including idenification of residents needs/issues and arrange for appropriate social support services.

-Provide assistance to residents and/or their responsible parties in obtaining financial services, as appropriate.

-Ensure that social services are coordinated with the other facility services.
