GCHS is seeking a Shared Services Patient Education and Community Referral Coordinator who will serve as a key liaison among regional shared services programs, local communities, and referring providers across Western Kansas. This role is responsible for coordinating patient education initiatives, strengthening referral networks, promoting oncology service awareness, and leading community and provider outreach efforts to improve access to oncology, surgery, and other outreach specialty services.The Coordinator plays a critical role in enhancing patient navigation, increasing early detection and treatment engagement, and supporting the overall growth and visibility of the regional oncology program Essential Duties and Responsibilities include the following. Education and/or Experience
Bachelor’s degree in healthcare administration, public health, communications, marketing, or related field preferred. Minimum of 2-4years of experience in healthcare outreach, patient education, marketing, or care coordination preferred. Experience in oncology, rural healthcare, or population health preferred.

Headquartered in Tribune, Kansas Greeley County Health Services is a rural health system serving western Kansas. GCHS boasts a 25-bed critical access hospital, 16-bed Long Term Care Unit, and 2 Family Practice Clinics located in Tribune and neighboring Sharon Springs in Wallace County. Serving multiple locations, travel is required for this position.
