Join Our Team as a Patient Engagement Pharmacist!

The Patient Engagement Pharmacist is responsible for improving patient outcomes through proactive medication management, education, and engagement strategies. This role focuses on building strong pharmacist-patient relationships, enhancing medication adherence, addressing barriers to care, and collaborating with interdisciplinary teams to support patient-centered, value-based care initiatives. This role will also be required to cover occasional shifts in the retail pharmacy.

Key Responsibilities:
- Provide exceptional customer service to patients and staff.
- Develop and maintain therapeutic relationships with patients through in-person, telephonic, and digital outreach.
- Provide individualized medication education, counseling, and motivational interviewing to support informed decision-making.
- Address patient concerns related to medication access, adherence, side effects and health literacy.
- Conduct comprehensive medication reviews (CMRs) and medication reconciliation.
- Work with clinicians to help contain cost of care, recommending covered medications to manage medical loss ratio.
- Collaborate with physicians, nurses, care managers, social workers, and other healthcare professionals to support coordinated care plans.
- Support quality measures (e.g., adherence metrics, start rating, value-based performance indicators).
- Ensure proper storage and handling of medications.
Ideal Candidates Will Have:
- Doctor of Pharmacy (PharmD) from an ACPE-accredited school of pharmacy.
- Active Ohio pharmacist license in good standing.
- Experience in patient-facing clinical pharmacy, ambulatory care, managed care, or specialty pharmacy.
- Strong communication, counseling, and interpersonal skills.
