Job Description

This is a temporary position, with the possibility of becoming a permanent position

Job Summary:

The Health Navigator oversees the Remote Patient Monitoring (RPM) application alerts, documenting the alert type and the corresponding response within the Electronic Medical Record (EMR). While the Health Navigator manages initial oversight, a program nurse is always available for clinical support.

Duties and Responsibilities:

Initiates outreach and successfully engages with patients enrolled in health monitoring programs.

Completes initial intake, including a comprehensive assessment of social needs, functional assessment of the patient in the home setting.

Monitor all alerts.

Answer, direct, and follow up on incoming calls

Review and maintain daily wellness call schedule.

Receive and perform wellness calls and document all information obtained on the call in the electronic medical record (EMR).

Support participants physical and emotional wellness with a holistic health approach.

Communicate concerns, changes in condition and all medical complaints to the Registered Nurse.

Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved.

Collaborates with nurse care managers, physicians, other care team members (Medical Assistants, Nurses, etc.), hospitals, partner agencies, to improve patient care.

Obtains releases, gathers PHI from outside providers involved in patient care as needed and processes documents according to program requirements.

Utilizes a multi-disciplinary team approach to address opportunities to plan and coordinate care.

Utilizes Motivational Interviewing skills and other patient engagement techniques with patients and caregivers.

Identifies and utilize culturally sensitive approaches and community resources.

Provides training to other team or practice staff as needed.

Attends required training and collaborative sessions as scheduled.

Performs other related duties as requested and serves on various system wide committees.

Requirements:

High School degree or GED required; Medical Assistant certification preferred.

Minimum two years required of training and skills with an interest in healthcare (pre-nursing/med) and willingness to learn and remain flexible in the changing healthcare environment.

Strong clinical background.

Must have a strong understanding of medical terminology.

Must possess excellent computer skills, especially Microsoft Office suite.

Bilingual (Spanish or Portuguese) desired, but not required.
