The Pediatric Care Management Intake Clinician (RN) leads clinical intake for pediatric patients referred to complex care or care management services. This role triages referrals, verifies eligibility, performs nurse-led intake assessments, risk stratifies medically and socially complex children, initiates family-centered care plans, and coordinates timely handoffs to assigned Pedi iCMP Care Manager. The RN partners closely with social work, pharmacy, CHWs, and CRS to reduce avoidable utilization and improve outcomes, experience, and equity.
Job Summary
Primary Responsibilities:
- Prepare patient/family for enrollment in program.
- Communicates with third party payers to obtain necessary authorization for reimbursement of services.
- Review cases with medical directors on challenging cases as needed.
- Defines care goals of patients by providing education, information, and direction to each individual and family.
- Maintain accurate, detailed reports and records.
Qualifications
Qualifications:
Associate's Degree Nursing required; Bachelor's Degree Nursing preferred.
Active Massachusetts RN licensure required.
2+ years of Pediatrics experience required.
Care / case management experience preferred.
Certification in Case Management (CCM/ACM) or Pediatric Nursing (CPN) preferred.
Experience with technology-dependent children, rare diseases, and medical home models preferred.
Bilingual ability and/or experience with interpreter services preferred.
Additional Knowledge, Skills and Abilities:
Strong clinical triage, assessment, and care planning skills (ie SMART GOALS)
Expertise working collaboratively with non-clinical roles (ex. Community Health Workers)
Familiarity with SDOH screening, community resources, and payer processes.
Excellent communication with families and interdisciplinary teams.
EMR proficiency; comfort with reports and registries.

Schedule and Working Model
Full-time Monday through Friday, standard business hours.
Work from home with the ability to attend in-person team meetings twice per month.
On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams video is required and must be accessed using MGB-provided equipment.
