Summary:With the support and in partnership with the E6 team, the E6 Peer Case Manager will support the E6 team successfully engage with people currently living on the streets, for the purpose of getting them matched to permanent, affordable housing. Peer Case Managers will walk the streets of Metro LA (Service Area 4) in teams to identify, engage and assist in housing homeless residents. These teams are part of a new large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). This is a LA County approved strategy to combat homelessness. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services.

Essential Duties and Responsibilities:

Conducting outreach to people experiencing homelessness living in Metro LA (Service Area 4)Assessing the housing needs of people living on the streetAssisting clients in the process of getting affordable housingAdvocating on behalf of client's needsDocument and account for time spent on client contacts and activities in client record in accordance with agency and funder standardsAttending team meetings and program-wide meetings dailyProviding support to nurses and social workers on the team with their client needsAccompanying clients to appointmentsProviding resource and referral information to clientsMaintain appropriate boundaries at all times

Qualifications

Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record preferredAn eagerness to provide top-notch quality services to individuals experiencing homelessness.Lived experience with homelessness, incarceration, or substance use.An understanding of and familiarity with homeless services and affordable housing in LA countyExperience in the fields of social work, public health or other types of outreachTraining in harm reduction, "housing first" principles and street-based outreach preferred

Preferred Qualifications

High School diploma or GEDPeer Counselor Training

Work Environment:

Field (may need to travel) and indoor office environment.Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.On occasion walk or drive to different local sitesWill necessitate working in busy and loud environmentsWill be exposed to elements like cold, heat, dust, noise and odorMay need to bend, stoop, twist and sit throughout the day
