Responsibilities

Provide direct patient care and carry out physician orders in compliance with state nursing practice standards.
Administer medications and treatments accurately and in accordance with facility policies.
Assess, monitor, and document resident status; promptly report changes in condition.
Collaborate with the interdisciplinary care team to develop and update individualized care plans.
Maintain professional, compassionate communication with residents, families, and colleagues.

Requirements

Current, unencumbered LPN licence in NM - required.
Minimum 1 year of experience in a long-term care or skilled nursing environment.
Familiarity with PCC (PointClickCare), MAR, and TAR documentation systems preferred.
Current BLS/CPR certification.
