Job Description
LPN: Licensed Practical Nurse
Per Diem | 4 Shifts per MonthRequirements: 2 weekend shifts per month, 1 summer and 1 winter holiday.
PAY RATE BETWEEN $26-30/HOUR + PICK UP PAY: $5/HOUR FOR WEEKDAYS AND $7/HOUR FOR WEEKENDS(based on experience and shift differentials)

LPN: Licensed Practical Nurse Requirements:
- Valid Licensed Practical Nurse (LPN) license in the state of Wisconsin.
- Current and valid CPR certification.
- Experience in long-term care is preferred.
- Basic computer proficiency.
- Ability to frequently push or pull objects weighing up to 60 pounds and occasionally lift and move objects weighing 100 pounds or more.
What you'll do as a LPN: Licensed Practical Nurse:
- Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies.
- Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements.
- Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans.
- Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.
