Responsibilities

Deliver skilled SLP services to residents in a post-acute/SNF setting.
Complete timely and accurate clinical documentation in compliance with facility and regulatory standards.
Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
Meet productivity standards while maintaining high quality of care.
Participate in patient/family education and discharge planning as appropriate.

Requirements

Valid NM state licence as a Speech-Language Pathologist (SLP) - required.
Graduate of an accredited programme in your discipline.
Minimum 1 year of clinical experience in SNF / post-acute care preferred.
Active NM Caregivers Registry account required.
Current BLS/CPR certification.
Strong documentation skills and ability to meet productivity targets.
