Overview

We are seeking a compassionate and creative Physical Therapist (PT) to join a specialized Memory Support Community in Falmouth, Maine, dedicated to providing individualized care for individuals living with Alzheimer's disease and other forms of dementia.

This unique rehabilitation environment goes beyond traditional therapy. The team incorporates residents' past and current hobbies, interests, and learning opportunities into daily treatment plans, creating therapy experiences that are meaningful, engaging, and connected to each person's life.

From art and music therapy to gardening and other purposeful activities, you'll have the opportunity to think creatively about rehabilitation while helping residents maintain function, engagement, dignity, and quality of life.

A Person-Centered Approach to Rehabilitation

The team uses its signature Compass program to develop individualized plans of care designed to maximize each resident's function and engagement.

Therapy goes beyond simply completing exercises. You'll have the opportunity to consider the person's environment, abilities, preferences, and daily routines when developing treatment strategies. Environmental accommodations and task modifications are incorporated to help residents participate more successfully in meaningful activities.

Care partners and families are also considered an integral part of the rehabilitation team, working alongside clinicians to help sustain quality of life and functional gains beyond the formal therapy period.

Professional Development

This position provides opportunities to expand your clinical knowledge through onsite training and evidence-based practice. Team members receive training in standardized assessments such as the Brief Cognitive Assessment Tool (BCAT), Global Deterioration Scale (GDS), and Allen Cognitive Level (ACL).

You'll also have support in applying evidence-based approaches informed by established researchers and clinicians in the field of cognitive and dementia care.

Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments

  • Develop individualized plans of care based on each resident's functional abilities, needs, and goals

  • Provide skilled therapeutic interventions to improve or maintain mobility, strength, balance, endurance, and functional independence

  • Incorporate residents' interests, routines, hobbies, and meaningful activities into therapy when appropriate

  • Use environmental accommodations and task modifications to maximize participation and engagement

  • Monitor resident progress and modify treatment approaches as clinically appropriate

  • Educate residents, care partners, and families regarding mobility, safety, exercises, and strategies to support continued function

  • Collaborate closely with nursing, therapy, care partners, families, and other members of the interdisciplinary team

  • Maintain accurate and timely clinical documentation

  • Promote dignity, safety, engagement, and quality of life throughout the rehabilitation process

Qualifications

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

  • Current Maine Physical Therapist license

  • Strong communication and interpersonal skills

  • Compassionate and patient-centered approach to care

  • Interest in working with individuals living with Alzheimer's disease and other forms of dementia

  • Ability to adapt treatment approaches to individual cognitive and functional needs

  • Strong clinical judgment and willingness to continue learning

  • Ability to collaborate effectively with residents, families, care partners, and interdisciplinary team members
