Overview
We are looking for a dedicated Physical Therapist (PT) to join a skilled nursing facility in Westbrook, Maine. This full-time opportunity offers 40 hours per week, Monday–Friday with a weekend rotation, providing a consistent schedule while allowing you to make a meaningful impact on patients throughout their rehabilitation journey.
The center provides post-hospital and short-term rehabilitation as well as long-term and respite care services, creating a diverse clinical environment where no two patient journeys are exactly alike.
The center has also earned the Skilled Nursing Facility Heart Failure Certification from the American Heart Association, reflecting a commitment to quality care and strong clinical standards.

Responsibilities
Conduct comprehensive physical therapy evaluations
Develop individualized plans of care based on patient needs and functional goals
Provide skilled therapeutic interventions to improve strength, mobility, balance, endurance, and independence
Work with patients recovering from hospitalization as well as those receiving long-term or respite care
Monitor patient response to treatment and modify plans as clinically appropriate
Maintain accurate and timely clinical documentation
Educate patients, families, and caregivers on therapeutic exercises, mobility, safety, and home programs
Collaborate with nursing, therapy, physicians, and other healthcare professionals
Promote patient safety and positive rehabilitation outcomes
Qualifications
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Current Maine Physical Therapist license
Strong clinical judgment and communication skills
Compassionate, patient-centered approach
Ability to work effectively within an interdisciplinary team
Strong organizational and documentation skills
