Full-Time Case Manager Registered Nurse (RN) Home HealthLocation: Coeur d'Alene, Idaho

This full-time opportunity is ideal for an experienced RN who thrives in a patient-centered, independent environment and is passionate about delivering high-quality care in the home health setting.
Position Overview The Case Manager RN will coordinate and deliver comprehensive home health services, ensuring continuity of care for patients in their homes. This role involves developing individualized care plans, collaborating with interdisciplinary teams, and supporting patients and families throughout their healthcare journey.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans*
- Coordinate and manage patient care across the continuum, ensuring quality and efficiency*
- Provide skilled nursing care in accordance with physician orders and established protocols*
- Monitor patient progress and adjust care plans as necessary*
- Educate patients and families on disease management, medications, and care techniques*
- Collaborate with physicians, therapists, social workers, and other healthcare professionals*
- Maintain accurate and timely documentation in compliance with regulatory standards*
- Ensure compliance with all state, federal, and agency guidelines
Education and Certification Requirements
- Active Registered Nurse (RN) license in the state of Idaho (or compact license)*
- Associate or Bachelor of Science in Nursing (ASN or BSN) required*
- Minimum of 12 years of nursing experience; home health or case management experience preferred*
- Current BLS (Basic Life Support) certification*
- Valid drivers license and reliable transportation
Preferred Qualifications
- Strong clinical assessment and critical thinking skills*
- Excellent communication and interpersonal abilities*
- Ability to work independently and manage time effectively*
- Experience with electronic medical records (EMR) systems*
- Knowledge of Medicare/Medicaid regulations in home health
