Physical Therapist Assistant (PTA) – Full-TimeCivita Care Center at Sheridan Woods | Bristol, CT$33–$36/hour | Full-Time | BenefitsPost Rehab is seeking a Physical Therapist Assistant (PTA) to join the rehabilitation team at Civita Care Center at Sheridan Woods. This role offers the opportunity to provide hands-on therapy while helping residents build strength, improve mobility, and work toward greater independence in a skilled nursing and post-acute care environment.What You'll DoProvide physical therapy treatments under the direction of a licensed Physical TherapistAssist residents with therapeutic exercises, gait training, balance, transfers, and functional mobility

Carry out individualized treatment plans based on resident needs and rehabilitation goals

Monitor resident response to treatment and communicate progress to the supervising PTReinforce exercises, mobility techniques, and safety strategies with residents and caregivers

Maintain accurate and timely clinical documentation

Collaborate with PT, OT, SLP, nursing, and other members of the interdisciplinary care team
