Physical Therapist Assistant With Post Rehab Services

Post Rehab Services at Civita Care Center at Milford is seeking a licensed Physical Therapist Assistant to join our therapy team in deliveringpatient-centered rehabilitation services at our skilled nursing facility.

In this role, you will work under the supervision of licensed Physical Therapists to implement evidence-based treatment plans that support residents in achieving optimal mobility, function, and independence.

Primary Responsibilities:

• Execute individualized treatment programs as directed by supervising Physical Therapists, including therapeutic exercises, gait training, transfer techniques, balance activities, strength training, and modalities to improve strength, range of motion, coordination, and pain management.
• Provide hands-on assistance and education to residents, promoting safety, functional independence, and participation in activities of daily living.
• Monitor resident responses to treatment, document progress accurately in compliance with Medicare and regulatory guidelines, communicate observations to the supervising PT, and collaborate with interdisciplinary team members to ensure comprehensive care.
