Job Description:
At Elara Caring, we care where you are and believe the best place for your care is where you live. As a Physical Therapist, youll provide high-quality care to patients in their homes. You will evaluate functional needs and outcomes, design and implement care plans, and consult with other specialists to ensure optimal patient outcomes.
- Promotes Elara Carings philosophy, mission statement, and administrative policies to ensure quality of care.
- Provides direct patient care and consults with other specialists.
- Designs and implements care plans incorporating participation by significant others when appropriate.
- Documents evaluations, treatment goals, and plans, maintaining prescriptions and signatures.
- Directs and assists support staff to achieve patient functional goals.
- Provides leadership and acts as a resource to promote physical therapy in the community.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare facility.
- Ability and willingness to travel within branch/office coverage area (~50%).
- Must have a dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
- Able to lift 50100 lbs. and work irregular hours if applicable.
