Responsibilities
Assess and treat patients with movement, strength, and postural concerns to improve function.
Develop personalized treatment plans aligned with patient goals and progress.
Document treatments, progress notes, and outcomes in the electronic health record.
Monitor patient outcomes using standardized measures and adjust plans accordingly.
Ensure compliance with clinical guidelines, safety standards, and regulatory requirements.
Collaborate with clinicians and therapists to coordinate care within the team.
Educate patients and caregivers on exercises, home programs, and self-management strategies.
Maintain high-quality patient care while achieving or surpassing performance targets.
Demonstrate growth mindset by pursuing continuing education and certification opportunities.
Participate in pipeline-driven territory planning to optimize patient referrals and access.
Represent the organization in community outreach and professional networking events.
Experience/Qualifications
Active Physical Therapy license valid in the applicable state.
Assess movement, strength, and posture to develop personalized plans.
Apply standardized outcome measures to monitor progress.
Ensure compliance with clinical guidelines, safety standards, and regs.
Meet or exceed performance targets using pipeline-driven referrals.
Document treatments and outcomes in the electronic health record.
Collaborate with clinicians to coordinate patient care within the team.
Educate patients and caregivers on home programs and self-management.
Demonstrate ongoing professional development and pursue certifications.
Represent the organization at community outreach and professional events.
