Overview

We are seeking a licensed Physical Therapist to provide high‑quality home‑based rehabilitation services across Cambria, Northern Cambria, and Indiana County. This role supports clinical care delivery within a home health environment and requires strong clinical judgment, autonomy, and excellent patient‑facing communication skills.

Mileage reimbursement is provided.

Key Responsibilities

• Deliver physical therapy evaluations and treatments in patient homes.

• Develop individualized care plans aligned with patient goals and clinical best practices.

• Document all care activities in accordance with regulatory and organizational standards.

• Collaborate with interdisciplinary care teams to ensure continuity of care.

• Participate in weekend/on‑call rotation (approximately every 8 weekends; final schedule discussed during interview).

Required Qualifications

• Master’s degree in Physical Therapy (or higher).

• Active Physical Therapist license in the state of Pennsylvania.

• Professional certification(s) as required for practice.

• Ability to meet all occupational health screening requirements.

• Valid driver’s license and reliable transportation.

• Weekend/On‑Call: Rotational, every 8 weekends
