KeystoneCare is a nonprofit provider of in-home healthcare services in the Hospital & Health Care sector, supporting patients and families with home health, hospice, palliative medicine, and private duty homecare. As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. Responsibilities

Provide personalized rehabilitation services to patients in their homes.
Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
Collaborate closely with a multidisciplinary team to support patient care and recovery.
Guide patients through their recovery journey with compassion and professionalism.
Help restore patients' independence and well-being through high-quality therapeutic care.

Requirements

Graduate of an accredited Physical Therapy program

Current Pennsylvania Physical Therapy License
Minimum two years of physical therapy experience in Home Health or similar settings
Experience treating geriatric patients in a home health or similar care setting
Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
Experience providing care in hospice-adjacent contexts
Valid Driver’s License and reliable vehicle
Proof of Auto Insurance
Strong communication skills for effective collaboration with patients, families, and the care team
Excellent organizational abilities and experience managing a diverse caseload
Eligible to work in the United States
Experience with Matrix Care is a plus.
