Job Description

Pride Health is hiring a Physical Therapist to support our client's healthcare facility in Russellville, AR. Interested? Apply Today!Job Details: Schedule: 8 AM-4:30 PMLocation: Russellville, AR Job Type: Contract Contract Length: 13 weeks (possibilities of extension) Pay Range: $52/hr- $56/hr*Pay offered is based on experience, expertise, credentialing, and education. Key Responsibilities:

Perform comprehensive patient evaluations and develop individualized plans of care
Provide skilled physical therapy services in patients’ homes
Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on exercises, safety, and ongoing care
Maintain timely and accurate clinical documentation in compliance with regulations
Collaborate with physicians, nurses, and interdisciplinary team members
Ensure compliance with Medicare guidelines and home health regulations

Preferred Qualifications

Experience with OASIS documentation Familiarity with home health EMR systems

Requirements:

Education:
License(s): Degree in Physical Therapy (PT) Active Physical Therapist license
Certification(s): BLS
Years of experience: 1 year
Skills/Specialties/Technology: Home Health
