Southern Oregon | Medford & Grants Pass

$55–$65/hour |$7,500 Sign-On Bonus (Full-Time)

Full-Time | Part-Time | PRN

  • Providing skilled PT care in patients’ homes

  • Completing admissions and routine visits

  • Developing and carrying out individualized care plans

  • Collaborating with nurses, therapists, and providersParticipating in interdisciplinary team meetings(The formal job description comes later—this is about the experience.)

  • Oregon PT license (or ability to obtain before start date)

  • 1–2 years PT experience (home health preferred, not)

  • Current CPR certification
