Role SummaryThe Physical Therapist is responsible for planning, organizing, and delivering high-quality physical therapy services to patients across a variety of settings. This role focuses on assessment, treatment, education, and collaboration to promote independence, safety, and enhanced quality of life. The ideal candidate is compassionate, skilled in rehabilitation strategies, and motivated to make a meaningful impact in the lives of older adults and individuals with complex needs.

Duties and Responsibilities

  • Provide exceptional physical therapy care as an active and collaborative member of the team.

  • Conduct in-person assessments for patients at initial enrollment and as needed, based on team determinations.

  • Evaluate and recommend adaptive and durable medical equipment, specialized footwear, and prosthetics; assess home safety and mobility, recommending modifications and equipment as appropriate.

  • Design and implement physical therapy treatment programs consistent with professional standards and organizational policies.

  • Deliver therapeutic interventions aimed at achieving individualized goals; collaborate with the team on patient care planning.

  • Accurately document evaluations, assessments, treatment plans, and outcomes in a timely manner.

  • Measure and document treatment effectiveness, revising plans as needed.

  • Educate and train patients, families, and caregivers on home programs, techniques, and equipment use to support ongoing recovery and independence.

  • Provide training to staff on body mechanics, equipment use, safe transfer techniques, and methods to promote mobility and safety in daily activities.

  • Deliver care and services in the clinic, center, contracted facilities, and patient homes in accordance with the individualized plan of care.

  • Support patients in achieving the highest possible level of independence and functionality.

  • Foster positive relationships with colleagues, patients, families, and community partners through effective communication and collaboration.

  • Contribute professional insight to the team to support comprehensive care planning and understanding of rehabilitation needs.

  • Participate in daily team meetings, communicating changes in patient status and contributing to coordinated care planning and delivery.

  • Step into other professional team roles, as appropriate, based on licensure and competencies.

  • Engage actively in quality improvement initiatives and professional development activities.

  • Attend organizational and community meetings related to patient care, operations, and ongoing education.

  • Complete all clinical documentation in compliance with organizational and regulatory requirements.

  • Adhere to all policies, procedures, and safety standards, including OSHA guidelines.

  • Maintain patient confidentiality and uphold privacy standards.

  • Ensure a safe work environment and follow all infection control and safety protocols.

  • Participate in required staff trainings, continuing education, and professional development.

  • Maintain current professional licensure, certifications, and affiliations.

  • Perform other duties as assigned.

Required Qualifications

  • Advanced degree in Physical Therapy from an accredited institution.

  • Current, active license to practice Physical Therapy in the state of employment.

  • Valid driver’s license, reliable transportation, and proof of auto insurance.

  • Minimum of two (2) years of experience working on a collaborative clinical team in a hospital, nursing home, or community-based setting.

  • Medically cleared for communicable diseases and current on all required immunizations prior to patient contact.

  • Successful completion of a comprehensive background check, including criminal history, license verification, education verification, and drug screening.
