This full-time opportunity offers the chance to provide individualized care while helping residents improve mobility, rebuild strength, and work toward greater functional independence.What You'll DoComplete physical therapy evaluations and develop individualized plans of care

Provide skilled treatment focused on strength, gait, balance, transfers, endurance, and functional mobility

Establish measurable rehabilitation goals and track resident progress

Adjust treatment approaches based on clinical findings and response to therapy

Educate residents and caregivers on exercises, mobility techniques, and safety strategies

Maintain accurate and timely clinical documentation

Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team

QualificationsActive West Virginia Physical Therapist license required

Graduate of an accredited Physical Therapy program

Strong clinical judgment, communication, and teamwork skills

Skilled nursing or rehabilitation experience preferred, but not required
