- Shift / Schedule: Day (5x8 Hours), 8:00 AM 4:30 PM
- Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day. System does a good job grouping clients, minimizing drive and windshield time.
- Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.
Job Summary
This is a Home Health Physical Therapist position serving West Portland, OR and surrounding areas. The role is based within a home health setting and involves patient visits in the community, with the facility grouping clients to minimize drive and windshield time.
Key Responsibilities
- Provide skilled physical therapy services to home health patients in West Portland, OR and surrounding areas
- Visit up to 6 patients per day; 30 productivity points standard (1:1 ratio)
- Complete Oasis assessments as required Oasis experience is required; please call out if traveler does not have Oasis experience
- Chart using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)
- Travel independently to patient homes; traveler must have their own transportation
- Manage a daily service area with an occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day
Required Skills
- Oasis experience required
- Own reliable transportation required
- Ability to manage home visit caseload of up to 6 patients per day
Education Requirements
- Oregon (OR) Physical Therapist License required
- BCLS certification required
Required Experience
- Minimum 1 year of skilled home health experience required

- Attire / Scrub Color: Business casual; no jeans; scrubs allowed but not required (no specific color)
- Parking Cost: Free
- Mileage Reimbursement: IRS Standard
- Facility requires primary series COVID vaccination in addition to one booster
- This facility cannot accommodate 4x10 or 3x12 schedules
- No on-call required
- No floating between like units required
- Block schedule not available
- Weekend work not required
Important Notes
- Locals accepted; considered "local" within a 50 75 mile radius; locals accepted at the same rate
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
- RTO Restrictions: Confirm with hiring manager
- First-time travelers accepted
